If this will be your first visit to Christian-Counseling-Solutions, there are several forms to fill out before your first appointment. If you have a personal computer with internet access and a personal email address, please contact me for information on how to fill out and sign the documents electronically.
Otherwise, you may download, print, and fill out the forms listed below and bring them with you to the appointment. Lastly, there will be paper copies at the office for you to fill out if necessary.
